We have now concluded an extensive investigation and have produced an incident report, investigating the root cause of the network failure, corrective actions taken and a preventive action plan.
Please find attached the Incident report for the network outage which resulted in several affected services on 10th November 2014.

In the last hour, some of you would have experienced a brief outage. Unfortunately, this was caused by an unauthorised change. We immediately rectified the situation and services are now fully restored. Please accept our apologies for the inconvenience caused – a full investigation is to be carried out in the next 5 days. Lessons learnt will be published shortly afterwards.
